Volunteer / Self-employed Teacher / Tutor / Coach / Governor Application Form

In order to submit this application form, you will need:

  • Your Passport number, National Insurance number, Driving Licence number, and the expiry date of your permit/VISA (if applicable).
  • Details of any qualifications related to the role (e.g., music exams, coaching qualifications, accounting qualifications etc.), including when and where obtained.
  • Full history of any previous roles in which you worked or volunteered with children.
  • Contact details for 3 referees (2 professional and 1 character reference).
  • A supporting statement (cover letter explaining your interest in the role and why you are a suitable candidate).

We anticipate that, if you have all of the above ready and to hand, this application form should take approximately 30 minutes to complete.

All applicants must complete this section. Please provide details of qualifications / certificates relevant to the role you are applying for.

For each entry, all columns must be completed. Please list in date order, with the most recent first.

Working with children

Please give details if you have previously worked with children on either a voluntary basis, or in paid employment (most recent first). If you have worked/volunteered in a school, please include the school's details. Where possible, please state the full date you started and ended your employment.

If you have not worked with children in the past, please enter N/A (not applicable) into each box.

References

Please provide three referees. One referee should be your current or most recent employer, the second should be a previous employer and the third should be a character reference.

If you are not currently working/volunteering with children but have done so in the past, one referee must be from the organisation for whom you were most recently working/volunteering with children.

Please note: references will not be accepted from relatives or from referees writing solely in the capacity of friends. If shortlisted, references will automatically be sought before your interview, and you will be responsible for their receipt by More House School at least one day before the date of interview.
Please note: If an exceptional reason exists where you do not wish your current employer to be approached before interview, it may be agreed that an additional reference from a role in which you have previously worked with children will be sought, until the reference from your current employer can be obtained.
We reserve the right to contact any of the above referees to to verify details given.

Supporting Statement

Please give a brief explanation a to why you are kindly offering your services to the school to work as a peripatetic teacher, after school instructor, or volunteer. Please also advise the times you are available.

Declaration

The amendments to the Exceptions Order 1975 (2013) provide that certain spent convictions and cautions are ‘protected’ and are not subject to disclosure to employers and cannot be taken into account. Guidance and criteria on the filtering of these cautions and convictions can be found on the Disclosure and Barring Service (DBS) website.
